Enhanced Care Management (ECM)

Enhanced Care Management (ECM) provides comprehensive, whole-person care coordination for individuals with complex physical, behavioral, and social health needs. Working closely with Inland Empire Health Plan (IEHP) and local providers, our dedicated care team coordinates your health, behavioral care, and social resources so you have continuous support every step of the way.

Looking for clinical therapy or psychiatry? Direct licensed clinical care (individual, couples, family, and child therapy, psychiatry, TMS, and Qb testing) is provided by our clinical partner, Renewing Hope Family Counseling Center at renewinghopeinc.com . We can also assist you with sliding-scale fee discount applications.

We connect you with primary care doctors, behavioral health specialists, mental health therapy, psychiatry, and oral health providers to ensure seamless, continuous care.

Our team assists you with understanding and navigating health plans (IEHP / Medi-Cal) and keeping the essential benefits you are eligible for.

We assist with scheduling medical visits, arranging transportation to appointments, medication management, and post-discharge follow-up care after hospital or crisis stays.

We bridge the gap to essential social services including housing navigation, emergency shelter referrals, food resources, and local community assistance programs.

Who qualifies

Enhanced Care Management is available at no cost for eligible Inland Empire Health Plan (IEHP) members, including individuals experiencing homelessness with complex needs, frequent hospital or ER visits, serious mental illness or substance use challenges, youth with complex health needs, or those transitioning from care facilities or incarceration. For non-IEHP individuals, our sliding-scale charity care program ensures no one is turned away for inability to pay.

  3. 3 We verify your IEHP / Medi-Cal coverage or sliding-scale eligibility.
  4. 4 Your dedicated care coordinator builds your personalized care plan and begins connecting you to services.

Fees

Cost should never be the reason you don’t reach out.

Your fee is based on your household income and family size, not on what you can prove you can pay. No one is turned away for inability to pay.

How the sliding scale works

  1. 1 Download and complete the Sliding-Fee Discount Program application (English or Spanish).
  2. 2 Provide household income verification (recent pay stubs, W-2, tax return, or benefits eligibility letter) and family size.
  3. 3 Our intake team reviews your application and confirms your discounted fee tier within 2 business days.
  4. 4 Your discounted fee schedule is applied to your counseling and care sessions at Renewing Hope.

If you cannot pay anything right now

Apply anyway. We hold a charity-care fund for community members in acute financial hardship. Use the contact form or call us at (951) 465-3665 , and we will work with you.

Common questions

Enhanced Care Management (ECM) is a comprehensive Medi-Cal benefit provided at no cost for eligible IEHP members. A dedicated care coordinator works closely with you and your providers to manage medical visits, mental health support, medications, and social resources like housing and food.

Direct clinical therapy, marriage & family counseling, child therapy, psychiatry, TMS, and Qb testing are provided through our clinical partner, Renewing Hope Family Counseling Center (RHFCC). Renewing Hope provides a sliding-scale discount based on household income and family size so that financial hardship never prevents anyone from receiving care. You can download the application in English or Spanish, provide proof of income, and our team will determine your discounted rate.

The mobile mental health and shower unit is operated under our trademarked partner organization, Move With Hope™. Visit movewithhope.org for mobile stops, community outreach requests, and mobile sponsorship details.
